Browser storage
This version saves records in your browser’s local storage. It does not send them to Degree Mantra, a university, an email service or a central customer database. The person using the same browser profile can access and download these records from the lead-records page. Avoid entering personal information on a shared device.
Consent
The form includes the requested contact-consent wording and links to the Amity Online privacy policy. That external policy belongs to Amity Online; it does not mean this independent site is operated by or sends information to Amity Online.
Your choices
You can close the guidance popup without submitting a form. To remove locally saved data, clear this site’s storage in your browser settings. You may first export your records from Lead records.
